When my pop was staying here, he almost died because they didn’t care if he ate or not and didn’t try to find out why. It turned out he had 2 abseses in his abdomen due to his surgery. He would have died if I had not sent by ambulance to the hospital.

MY husband has been in POST ACUTE for about two weeks. Ninety-Eight percent of the staff is very friendly. They are constantly checking on my husband to make certain that he is ok or if he needs anything. My husband is an Air Force Veteran. I like that Post Acute recognizes the sacrifices of service members such as my husband. Thank you Molly, Thank you Christian. Keep up the great work! FrankieJ

I’d like to give a review of the Sacramento Post Acute facility on Hemlock Ave, in Sacramento. The people are what make this such a good place to recover in. Julie Swift on intake, Celly the CNA was available at the push of a button. Other staff were always there and very helpful. PT folks, Morgan, Adriana, Kimi, Stephanie, and Gwen were there everyday to help me gain the usage of my left knee. Excellent folks and they followed plans to get me on my way as soon as possible. I can’t think of anything that would make me change my mind on the quality of the care I received there.

The home was clean and staff friendly. A relief.

I was greeted by Cindy who walked with me to visit my friend. He wasn’t in his room! She offered to help me locate him. Well, it ended up he was at his doctor’s appointment. I truly appreciated her kind spirit and her willingness to assist me. Thank you Cindy!

Morgan and Adriana have been working with my sister doing physical therapy and they are amazing. She went from bedridden to sitting up and in a wheelchair chair in a matter of days! Thank u so much!

We had such a great experience here when my 90 year old father needed help. The staff was great and responsive. Only exception was the travel/appointment coordinator…a lot of times there was confusion in communication with her. But overall a really excellent place!

The faculty and staff were absolutely wonderful. Everyone was professional, compassionate, and genuinely dedicated to providing the best care and support possible. They treated me and my husband with kindness and respect, always taking the time to listen and make sure my husbands needs were met. It’s clear they truly care about the people they serve, and I’m very thankful for the positive experience we had.

I thought the staff was very good. The OT and PT was excellent. My father was there for 10 days recovering from a hip replacement. We were grateful for the staff.

I give three stars only because of the PT, OT, they did an excellent job with my mother, the caseworker is another story, In my opinion if you place a family member in this facility, make sure you have an advocate, there was such bad communication between the caseworker I’m still having issues with my mom who left the facility. Stay on top, check all meds being administered, I was baffled they were giving my mom depression meds on a stroke patient. A lot of times I would come there and my mom would be soiled, I would have to get her cleaned up. Stay up on the communication in and out.

First full day in and I found everyone to be helpful, patient, informative and very friendly. They were knowledgeable and the physical therapist was very encouraging.

This building has been amazing! The staff here is very friendly, I definitely recommend to bring your loved ones here , the meals were good, and always received snacks when needed, the rehab therapy group helped me recover, I’m ready to go home today to my wife, thank you Sac post acute!!

I’ve been to three different facilities in the past, and this has to be one of the best ones! The staff is amazing and knowledgeable and know how to treat the residents here, the therapy rehab is amazing, and are helpful they really try to get us mobile, the food here is amazing as well, this place is motivating! Nursing department do a great job and work well to help us.

my husband has been in this post acute here for a few weeks, he was in 2 others and this one is far better than the other sub or post acute facilities!! friendly staff and nurses, respiratory therapist were all very knowledgeable and helpful, They all took good care of him.

The staff is very friendly, they quickly take care of the patients needs. They introduce themselves to friends and family. I would recommend this skilled nursing facility.
